Our online patient transfer platform will be available soon, providing a simple way to submit patient details and arrange transfers to our team.
Our team is here to help. While our online transfer platform is being finalised, please call us ahead of any patient transfer to let us know your patient is on the way and discuss their needs with our team.
Please also complete a Patient Transfer Form, which can either accompany your patient or be emailed to us in advance, along with the patient’s clinical history.
